The Croatian striker opted to leave the German champions, who recently signed Robert Lewandowski on a free from rivals Dortmund.
Mandzukic has signed a four-year deal with Spanish champions Atletico.
The 28-year-old scored 33 goals in 54 Bundesliga games for Bayern, in two seasons at the club.
He has been brought in to replace Diego Costa, Chelsea's new £32m man, who scored 36 goals for the club last season.
"We thank Mario for two great years here in Munich," said Bayern deputy chief executive Jan-Christian Dreesen.
Mandzukic was at the World Cup with Croatia, scoring twice against Cameroon, but he could not help his side get out of the group stage.
