The Premier League club have agreed a £100,000 move, that could rise to £400,000 with Newport County, and will head to Carrington for a medical, according to the Daily Mail.
Poole has started four matches for Terry Butcher's team this season, who are bottom of the Football League with only a single draw in their first five matches.
He has seen interest from numerous clubs, having spent time training with Manchester United and previously Liverpool, while Fulham and Hull City had bids rejected by the Welsh club.
Poole is not on a professional contract at Rodney Parade but has been a member of the first-team since making his debut at just 16 years and 94 days last September.
