Manchester City captain Kompany pulled up just minutes into the second-leg tie when dealing with an attack from the visitors to the Etihad.
It was immediately clear that Kompany had seriously hurt himself with the Belgium international dropping to the floor and sitting on the turf with a resigned look on his face. The injury appears to be related to his calf and will almost certainly mean he misses the game against Manchester United. How much of the season run-in it will affect is unclear.
He was immediately replaced with Eliaquim Mangala coming on.
Things got worse for Manchester City when Nicolas Otamendi had to be taken off after hurting himself in a collision. The Argentine's injury looked less serious with the defender able to continue for a few minutes before being replaced.
The injurie will be a major blow to Man City's hopes against near neighbours Manchester United at the Etihad this weekend. City's form with their captain is markedly better than without him.
Kompany has been beset by injury problems this term, playing just once in a three-month period spanning the Christmas holidays.
