Jose Mourinho's squad finally lifted the Premier League trophy on Sunday after the home win against Sunderland, after securing the title three weeks ago, prompting wild celebrations amongst players.
Four trophies will be on display for fans along the west London route, with the Academy joining the first-team squad on the route. The Premier League trophy and the League Cup will be on display as well as the FA Youth Cup and Uefa Youth League trophies.
The parade begins at 12pm at Stamford Bridge's west entrance, travelling along Fulham Road before turning right onto Hortensia Road, right again onto King's Road and then on to New King's Road.
The parade will finish at Eel Brook Common where the buses will stay for a short time, departing towards Parsons Green at about 1pm.
Big screens and a PA system will also be placed on Eel Brook Common to broadcast the parade - the park will have a viewing area for people with disabilities as well as toilets.
Road closures in the area have been in effect from 7.30am this morning.There is expected to be a knock-on effect for Chelsea in the surrounding areas as clearing the public, litter and barriers will take time.
