Following a goalless 120 minutes, the Premiership visitors edged ahead when they scored first from the spot before Shaka Hislop saved from Reading's James Harper.

But the Hammers' Jermain Defoe hit the woodwork and, with the scores level at 5-5, Scott Minto missed to leave Adrian Viveash to step up and fire home the winner for the Second Division team.

The Premiership leaders Bolton stumbled into the third round at the expense of First Division Walsall on a night of drama at the Reebok Stadium which saw the hosts edge home after extra-time.

Darren Wrack put Walsall ahead in the 65th minute only for Michael Ricketts to level, but the visitors then stunned the Reebok with two quick goals.

First, substitute Darren Byfield headed home seven minutes from time before Tony Barras put the Saddlers 3-1 ahead. Then, with Walsall poised to claim a famous win, the Japanese debutant Akinori Nishizawa slotted home deep in stoppage time for Sam Allardyce's side to take the match into extra time.

Henrik Pedersen's volley six minutes into extra time squirmed over the line, but still the drama was not over. The visitors were awarded a penalty after Colin Hendry upended Byfield but Barras saw his spot-kick saved by goalkeeper Steve Banks and Bolton held on.

At Spotland, the Third Division leaders Rochdale also went close to pulling off a giant-killing act before going down 6-5 on penalties to Fulham. The game ended 2-2 after 120 minutes, and Luis Boa Morte fired Fulham into the next round with the decisive penalty.

Peterborough almost pulled off an upset when they led Coventry City 2-1 at London Road with less than a minute to go when Lee Carsley took the tie into extra-time. The managerless Sky Blues came through 4-2 on penalties after the game stayed level at 2-2.

Birmingham, last season's beaten finalists, were back on the road to Cardiff with a 3-0 win at Bristol Rovers.

Southampton scored their first goals of the season in a 3-0 win at Brighton. James Beattie hit the first before the break and Anders Svensson added two more in the second half.

David Murphy scored on his debut for Middlesbrough and then paved the way for Szilard Nemeth to record his first for the club. Sam Parkin hit back for Northampton, but Mark Wilson took Boro to their first win of the season by making it 3-1.

Bruce Dyer bagged two goals in Barnsley's 3-1 win over Colchester. Marlon King's fourth goal of the season put Gillingham on the way to a 2-1 win over Millwall, while Tranmere thumped Preston 4-1 at Prenton Park.

Extra-time goals by Dave Watson and Shaun Smith, a penalty, gave Crewe a 2-0 win over Rushden & Diamonds, and more extra-time goals, from Paul Dickov and Darren Huckerby, gave Manchester City a 4-2 win over Notts County at Meadow Lane.
