The Chelsea captain left the field on a stretcher without about 20 minutes to go in the 4-0 Champions League win over Maccabi Tel Aviv.
Terry appeared to injure his left leg after a fairly innocuos challenge in the middle of the field.
Speaking after the match, Jose Mourinho blamed the state of the Israeli pitch - he also lost Ramires in training on Monday because of the poor surface.
"We already had one injury on this pitch in training with Ramires," he told BT Sport. "Another one tonight with John Terry.
"We are lucky it was just two.
"Of course he is a doubt [for the Spurs match]. I don't want to speak without having tests back in London.
"I think he is a really big doubt."
Mourinho could content himself with a win that moved Chelsea top of the group and needing only to draw their final group game, at home to his former club Porto, to confirm their place in the last 16.
Porto were beaten 2-0 at home to Dynamo Kiev in the other match in the group on Tuesday night, and Mourinho said: "(The win is) very important, especially with what happened in Porto. We really needed to win this match.
"It is also an important two victories in a few of days. I hope confidence comes. It was important because we went a lot time without consecutive victories. To do that is great."
Mourinho later added: "The surface was bad and dangerous. John has an ankle problem that we don't know yet the dimension of.
"For John to leave the pitch with the result 1-0 I know already that he is in trouble.
"My medical department is really good. John is a special guy, with a special desire to recover.
"But I think to leave the pitch with the result 1-0, I don't believe too much that he can play (against Spurs)."
