Karen McDougal said she slept with the US president “many dozens of times” after their relationship began in 2006 - three months after his wife Melania gave birth to their son Barron.
“I was attracted to him. He’s a nice looking man. I liked his charisma,” she told CNN, adding that she met him at the Playboy Mansion where Mr Trump was filming Celebrity Apprentice.
The ex-Playmate said their first sexual encounter was at the billionaire’s bungalow at the Beverly Hills Hotel.
“After we had been intimate, he tried to pay me, and I actually didn’t know how to take that,” she told host, Anderson Cooper. “I looked at him and I said: ‘That’s not me. I’m not that kind of girl.’”
She said was left ”crying in the backseat of a car” on the way home.
Afterward, she said she did not think she would see Mr Trump again, but agreed to go on further dates with him after he called her back and was ”very charming” and “sweet.”
She claimed they had unprotected sex many times during the affair, which she eventually ended because guilt was “tearing [her] apart”.
The model said she met Melania at the Playboy Mansion at a party for the airing of Celebrity Apprentice - a claim backed up by a photograph of Ms McDougal with Mr Trump, his wife, his daughter Ivanka, and other Playboy models.
Ms McDougal said the President once told her she was “beautiful like” his daughter.
She added that Mr Trump was very proud of Ivanka as he should be.”
She said: She’s a brilliant woman, she’s beautiful. He said I was beautiful like her and, you know, ‘you’re a smart girl’ and there wasn’t a lot of comparing but there was some, yeah. I heard a lot about her.”
Asked if Mr Trump ever said he loved her, McDougal answered: ”All the time. He always told me he loved me.”
She said she thought the affair would “maybe” lead to marriage.
Ms McDougal said she broke up with Mr Trump in April 2017 because she felt guilty about the affair.
Asked what she would say to Melania, she said: “What can you say except, I’m sorry? I’m sorry. I wouldn’t want it done to me.”
She said that she wanted to share her story “because everybody else is talking about it.”
The White House said Mr Trump denies having an affair with Ms McDougal.
The President already faces claims of an extramarital affair with adult film star Stormy Daniels in 2006.
Daniels, whose real name is Stephanie Clifford, received $130,000 (£92,000) from Mr Trump’s personal lawyer, Michael Cohen, to keep quiet about their alleged sexual relationship.
But she went public after claiming he had broken the non-disclosure agreement.
