The assault, caught on CCTV, happened in broad daylight on a pavement in the Prospect-Lefferts Gardens area of Brooklyn, not far from a police station.
The 71-year-old woman was knocked unconscious and was found to have suffered a broken jaw and cuts to her mouth.
The man is seen walking in the opposite direction to the woman. As he draws level he casually turns towards her and punches her in the head with his right hand, sending her sprawling to the ground. He then calmly walks away.
According to NBC New York, the woman came round and walked into a shop, telling staff she could not remember what had happened but thought she had fallen.
Anabelle Beckford told NBC New York: "When she first came in I was asking her, where were you - she didn’t even know where she was. I said, where did you fall. She said 'I don’t know, somewhere round here’."
Police are appealing for information about the suspect, who was wearing a grey sweatshirt and tracksuit bottoms. He was apparently seen in the area before the incident and police believe he may have mental health problems.
The attack happened at about 5.30pm on 19 September on New York Avenue.
