The man's car collided with another vehicle on the eastbound carriageway of the A50 near Stoke-on-Trent at 6.15 this morning.
He was being chased by police for having failed to stop.
The woman was taken to hospital, where she remains in a critical condition.
The passengers in the second vehicle suffered only minor injuries, and were taken to hospital as a precaution.
The dual carriageway between the Baths Road and Trentham Road junctions was closed in both directions for more than nine hours, according to the Stoke Sentinel.
A Staffordshire Police spokesperson appealed for witnesses, and said the incident would be investigated by police regulators.
"The Collision Investigation Unit are at the scene and we would ask for any member of the public with information about the incident to call 101 quoting incident number 177 of 15 April.
"Due to our involvement before the collision, we have voluntarily referred ourselves to the Independent Office for Police Conduct," the force said.
